10/2/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C greeted Client upon arrival to the home. QMHP-C arrived to the home to conduct an individual session with Client. QMHP-C informed Client that the purpose of the session was to focus on Client demonstrating effective communication skills. 2. Behavior: Client appeared to be unaware of how to control his feelings when becoming frustrated and angry. and needing some reassurance he would make progress with his goals if he applies what is being taught to him during sessions. 1.Intervention: QMHP-C addressed Client’s ability to get along with others and show age-appropriate interactions today. QMHP-C explained to Client the importance of being able to get along with his peers and using those techniques as discussed within past counseling sessions monitoring his impulsive actions. 2. Intervention: QMHP-C provided Client with anger management techniques to support with identifying his triggers and fuses. QMHP-C educated Client on coping skills, and conflict resolutions, and actively listened to Client communicate his feelings and current struggles such as his daily stressor. 1. Response: Client appeared to be receptive to QMHP-Cs’ interventions and services that was provided to him during the session. Client appeared to be unaware of how to control his feelings when becoming frustrated and angry. During the session, Client could identify coping interventions to reframe from unacceptable behaviors. 2. Response: When QMHP-C provided Client with a psychoeducation regarding his feelings of anger and how his feelings of anger may impact his behavior, mood, and disposition.
